I understand that in compliance with the Fair Credit Reporting Act this information will be used to obtain background information on myself and my company. I understand that the fee received for this investigation is non-refundable regardless of outcome.

I hereby consent to and authorize Better Consumer Choices LLC ("BCC") and/or its agents to: (a) conduct any and all investigations which, in BCC's sole opinion, are necessary in connection with this matter; (b) obtain consumer and other information as to my character, general reputation, credit capacity, personal characteristics and mode of living and (c) prepare any and all reports in respect of (a) and (b) above, (a), (b) and (c) hereinafter collectively referred to as "Investigation." I understand and agree that BCC will utilize the services of J. Mitchel Private Investigations to conduct the Investigation.

I further hereby consent to and authorize J. Mitchel Private Investigations and/or its agents to: (a) conduct any and all investigations which, in J. Mitchel Private Investigations's sole opinion, are necessary in connection with this matter; (b) obtain consumer and other information as to my character, general reputation, credit capacity, personal characteristics and mode of living and (c) prepare any and all reports in respect of (a) and (b) above, (a), (b) and (c) hereinafter collectively referred to as "Investigation."

I further authorize any person associated with any educational institution, any law enforcement agency (including but not limited to local, state, federal) any credit reporting organization, or any person who has knowledge of my character, work/business ethics/experience, criminal record, education or medical history, to release information to J. Mitchel Private Investigations", its employees and/or agents either orally or in writing. This Investigation may involve personal interviews with sources such as friends, associates, past employers and educational institutions. Public and private records may be used in this report, including but not limited to civil and criminal records, driving records, liens, judgments, bankruptcy, that are deemed by BCC in its sole opinion to have a bearing on my personal or business reputation.

I hereby release, indemnify and hold harmless BCC and J. Mitchel Private Investigations, their investigator(s) and agents from any and all claims and causes of action, all liability(ies) or damages resulting from the Investigation as well as any of its activities in connection with this matter. Further, I hereby release BCC and/or J. Mitchel Private Investigations, their employees, agents, investigator(s) from any claims for damages for any errors or inaccuracies in the acquisition or transmittal of information pertaining to the verification of my back ground and this Investigation.

I understand and agree that the fee I have paid for this Investigation is non-refundable regardless of the outcome of the investigation. I agree that BCC has the sole right at its sole discretion to accept or reject my application and I further agree that I will make no claims nor file any causes of action against anyone, including but not limited to BCC, its agents, investigator(s) or employees in respect of BCC's decisions with respect to my membership.
